Designed Wellness

  • NEW for 2022:
    • A guaranteed $25 incentive for any covered employee or spouse who completes the health assessment in 2022.
    • $10 BONUS incentive for anyone who registers their account in 2022 (if they had not registered previously).
  • Continuing:
    • Continue participating in healthy activities to earn more ticket entries into the quarterly raffle drawing for prizes from $50-$300.

Premier Plan

  • NEW for 2022:
    • My Care Checklist – earn points for completing age- and gender-appropriate preventive exams.
    • Ovia Health – a suite of apps to assist couples with fertility, pregnancy and parenting.
  • Continuing:
    • All covered employees and spouses can earn up to $300 per person in incentives for participating in 2022.
    • Earn $50 for completing the health assessment and $100 for completing the biometric screening.

Platinum (Formerly known as AHIP)

  • NEW for 2022:
    • Members have additional time this year to complete their AHIP requirements, with the compliance deadline moving to Nov. 30, 2022.
    • My Care Checklist – earn points for completing age- and gender-appropriate preventive exams.
    • Ovia Health – a suite of apps to assist couples with fertility, pregnancy and parenting.
  • Continuing:
  • All covered employees and spouses can earn up to $400 per person in incentives for participating in 2022.
  • Earn $200 for completing the AHIP compliance requirements (health assessment and screening or reasonable alternative, if applicable).
